Medicaid Service Coordination
Springbrook employs Medicaid Service Coordinators who work closely with adults and children with developmental disabilities and their families to obtain the services, outcomes and dreams the individuals desire. Medicaid Service Coordination is available to children and adults in our residential programs as well to children and adults living in the surrounding area. The main focus of this service is to provide linkage and referral services to individuals and their families on a long-term, on-going basis.
A Service Coordinator provides assistance in navigating regulatory systems such as the New York State Education Department (SED), Office of People With Developmental Disabilities (OPWDD), New York State Department of Family Assistance and Medicaid to assure that the people they serve have access to comprehensive medical, clinical, educational, vocational and residential services as needed. A Service Coordinator is an individual's and family's advocate, coordinating all team members in a goal-orientated service plan which ensures the best services for the individual.
Service Coordinators are very familiar with the process of assisting individuals and their families with setting up Medicaid and with obtaining other benefits such as Social Security.
